NOTICE OF PRIVACY PRACTICES
Effective Date: September 1, 2026
PLEASE READ THIS NOTICE CAREFULLY.
This Notice of Privacy Practices describes how Yes2Life Natural Health ("Yes2Life," "we," "us," or "our") may use and disclose your protected health information ("PHI"), how we protect your health information, your rights regarding your health information, and how you may contact us with questions or concerns.
Yes2Life Natural Health is committed to protecting the privacy and security of your health information.
This Notice applies to health information created, received, or maintained by Yes2Life Natural Health in connection with the health-care services we provide to you.
1. HOW WE MAY USE AND DISCLOSE YOUR HEALTH INFORMATION
We may use or disclose your PHI without your separate written authorization when permitted or required by federal or Missouri law.
Treatment
We may use or disclose your PHI to provide, coordinate, or manage your health care.
For example, we may use information from your health history, intake forms, examination, and treatment records to evaluate your health and provide chiropractic, naturopathic, nutritional, wellness, or other services offered by Yes2Life Natural Health.
We may also disclose appropriate information to another health-care provider involved in your care when necessary for treatment.
Payment
Yes2Life Natural Health is a cash-pay practice and does not bill health insurance companies.
We may use and disclose PHI as reasonably necessary to receive payment for services we provide.
We may accept payment by:
• Cash;
• Credit or debit card;
• PayPal; and
• Other payment methods that Yes2Life Natural Health may make available from time to time.
When using a third-party payment processor, we may disclose information reasonably necessary to process and document a payment.
We do not submit claims to health insurance companies on behalf of patients.
Health-Care Operations
We may use or disclose PHI as necessary for our health-care operations, including:
• Maintaining and managing patient records;
• Quality assessment and improvement;
• Staff training;
• Business administration;
• Legal and compliance activities;
• Auditing;
• Scheduling and appointment management;
• Information technology and electronic record systems; and
• Other activities necessary to operate Yes2Life Natural Health and provide services to our patients.
Appointment Reminders
We may contact you by telephone, text message, email, postal mail, or another communication method you have provided to us to remind you about appointments.
Appointment reminders are considered part of treatment and generally may be sent without a separate HIPAA authorization.
If you do not want to receive appointment reminders by text message, please contact us using the information provided at the end of this Notice.
Intake Forms and Patient Communications
Before your first appointment, Yes2Life Natural Health may send you patient intake forms by U.S. Postal Service mail.
We may also ask you to complete and bring printed copies of those forms to your first appointment.
We may use the contact information you provide to communicate with you concerning your appointment, intake process, treatment, billing, records, or other health-care matters.
When we mail health information or intake materials, we will use reasonable safeguards appropriate to the circumstances.
Individuals Involved in Your Care
When permitted by law, we may disclose relevant health information to a family member, close friend, caregiver, or another person you identify as being involved in your health care or payment for your care.
We may disclose information in circumstances where you agree or do not object when permitted by law.
Required by Law
We may use or disclose PHI when federal, state, or local law requires us to do so.
Public Health
We may disclose PHI for public-health activities when permitted or required by law, including activities related to preventing or controlling disease, injury, or other health conditions.
Health Oversight
We may disclose PHI to appropriate governmental agencies for health-oversight activities authorized by law, including audits, investigations, inspections, licensing activities, and disciplinary proceedings.
Legal Proceedings
We may disclose PHI when permitted or required by law in response to a court order, subpoena, warrant, discovery request, or other lawful process.
Law Enforcement
We may disclose PHI to law-enforcement officials when permitted or required by applicable law.
Serious Threats to Health or Safety
We may use or disclose PHI when necessary to prevent or lessen a serious and imminent threat to the health or safety of a person or the public, when permitted by law.
Workers' Compensation
We may disclose PHI as authorized by and to the extent necessary to comply with workers' compensation laws and similar programs established by law.
Business Associates and Service Providers
Yes2Life Natural Health may use third-party companies or individuals to perform services on our behalf.
These may include providers of:
• Electronic health-record services;
• Scheduling services;
• Information technology;
• Billing or bookkeeping services;
• Payment processing;
• Secure communications;
• Legal or accounting services; and
• Other administrative or professional services.
When required by HIPAA, we will enter into appropriate business associate agreements with vendors that create, receive, maintain, or transmit PHI on our behalf.
2. EMAIL MARKETING AND PROMOTIONAL COMMUNICATIONS
Yes2Life Natural Health may use email to communicate with patients about health-related information, educational materials, practice news, wellness information, events, services, or other communications.
Certain communications about health-related products or services may be permitted under HIPAA without a separate authorization. Other communications may constitute "marketing" under HIPAA and may require your written authorization.
When HIPAA requires an authorization for marketing, we will obtain the required authorization before sending the communication.
You may request to stop receiving promotional or marketing emails at any time by using the unsubscribe instructions included in the email or by contacting Yes2Life Natural Health directly.
Choosing not to receive marketing communications will not affect your ability to receive health-care services from Yes2Life Natural Health.
3. USES AND DISCLOSURES REQUIRING YOUR WRITTEN AUTHORIZATION
Certain uses and disclosures of PHI require your written authorization.
These may include:
• Certain uses or disclosures of psychotherapy notes, when applicable;
• Uses or disclosures of PHI for marketing when an authorization is required;
• Sale of PHI; and
• Other uses or disclosures for which federal or applicable state law requires authorization.
If you provide an authorization, you may revoke it in writing at any time, except to the extent that we have already taken action in reliance on the authorization.
4. YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION
You have the following rights concerning your PHI, subject to certain limitations established by law.
Right to Inspect and Obtain a Copy
You generally have the right to inspect and obtain a copy of health information that we maintain about you.
You may request your records in paper or electronic form, when available.
Missouri law also provides patients with rights concerning access to their medical records. (Missouri Revisor of Statutes)
We may charge fees permitted by applicable law.
Right to Request an Amendment
You may request that we amend health information that you believe is incorrect or incomplete.
Your request must be made in writing and explain why you believe the information should be amended.
We may deny an amendment request in certain circumstances permitted by law.
Right to Request Confidential Communications
You may request that we communicate with you about your health information by a particular method or at a particular location.
For example, you may request that we contact you at a particular telephone number, email address, or mailing address.
We will consider reasonable requests as required by applicable law.
Right to Request Restrictions
You may request that we restrict how we use or disclose your PHI for treatment, payment, or health-care operations.
We are not required to agree to every restriction request.
However, when you pay completely out of pocket for a health-care item or service and request that we not disclose information concerning that item or service to your health plan for payment or health-care operations, HIPAA generally requires us to honor that request unless disclosure is otherwise required by law.
Because Yes2Life Natural Health does not bill health insurance, this restriction may be less frequently applicable to our practice.
Right to an Accounting of Certain Disclosures
You generally have the right to request an accounting of certain disclosures of your PHI made by us during the six years preceding your request.
Certain disclosures are excluded from this accounting, including many disclosures made for treatment, payment, and health-care operations.
Right to Receive a Copy of This Notice
You may request a paper copy of this Notice at any time.
If this Notice is available on our website, you may also obtain an electronic copy from the website.
Right to Receive Notice of a Breach
You have the right to receive notice when a breach of your unsecured PHI occurs and notification is required by law.
5. OUR RESPONSIBILITIES
Yes2Life Natural Health is required by law to:
• Maintain the privacy of your PHI;
• Provide you with this Notice describing our legal duties and privacy practices;
• Follow the terms of the Notice currently in effect;
• Notify you as required by law if a breach occurs that compromises the privacy or security of your PHI; and
• Comply with applicable federal and Missouri privacy laws.
We reserve the right to change our privacy practices and this Notice.
If we materially change our privacy practices, we will revise this Notice accordingly.
The current version of this Notice will be available at our practice and, if applicable, on our website.
6. ADDITIONAL PROTECTION FOR CERTAIN HEALTH INFORMATION
Certain types of health information may receive additional protection under federal or Missouri law.
For example, certain substance-use-disorder records may be subject to additional federal confidentiality requirements under 42 U.S.C. § 290dd-2 and 42 CFR Part 2.
When applicable, Yes2Life Natural Health will comply with those additional requirements.
7. MISSOURI PRIVACY REQUIREMENTS
Yes2Life Natural Health will comply with applicable Missouri laws governing the confidentiality, access, release, retention, and disclosure of patient health information.
Missouri law provides patients with rights to obtain copies of certain medical records upon written request, subject to applicable limitations and fees. (Missouri Revisor of Statutes)
Where applicable federal and Missouri privacy requirements differ, Yes2Life Natural Health will comply with the requirement applicable to the particular circumstance.
8. YOUR RIGHT TO FILE A COMPLAINT
If you believe your privacy rights have been violated, you may file a complaint with Yes2Life Natural Health.
You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
You will not be retaliated against for filing a privacy complaint.
